**Position: Community Support Worker**

**Job Type**:Full-time (Minimum 38 hours per week)

**Duration-**at least for 2 years

**Salary range**:$77000 -$95000

**About The Role and Responsibilities**

Oasis Services Australia Pty Ltd is a registered NDIS provider based in Victoria, dedicated to delivering high-quality, person-centred care to individuals living with disabilities. Our mission is to empower our clients to lead fulfilling and independent lives by providing tailored support services that promote inclusion, dignity, and personal growth.

**Tasks Include**:

- Provide practical and emotional support to individuals and families, including referrals to available resources and emergency supplies, such as food vouchers.
- Assess individuals’ and families’ needs by planning, implementing, and evaluating relevant educational, training, and support programs.
- Evaluate community needs and resources related to health, welfare, housing, employment, training, and other essential services.
- Raise awareness in the community and advocate for groups requiring additional assistance to promote social inclusion and connectivity.
- Educate individuals to develop life skills that enable independent living, while informing them of available support resources.
- Identify and resolve immediate issues such as housing shortages, mental health crises, and other urgent health emergencies.
- Support individuals and families to actively participate in their local community and engage in meaningful activities that promote healthy and active living.
- Coordinate mobile voluntary services, including emergency food responses and delivery of supplies for community programs.
- Provide transport for individuals to attend appointments, programs, or recreational activities.
- Assist individuals and families with recovery planning, goal setting, and accessing mental health services.

**Qualifications and other requirements expected from a Community Support Worker**
- A Certificate III in Individual Support or Certificate IV in Disability, Individual Support, Community Services, or an equivalent qualification will be highly regarded or a minimum of three years of relevant full-time experience is considered in lieu of the formal qualifications.
- A recent minimum of one year of relevant full-time experience working similar role within the disability or aged care sector as a support worker
- Experience working with people living with Mental Health disorders highly regarded
- A valid driver license with your own reliable vehicle.
- Ability to work independently and without supervision.
- Current First Aid and CPR certification
- NDIS Worker Screening Check and Working with Children Check (or willingness to obtain)
- Current Police Check
- Strong interpersonal and communication skills
- Understanding of duty of care and person-centred approaches

Pay: $77,000.00 - $95,000.00 per year
