**ABOUT THE POSITION**

Support Worker is responsible for delivering support services aimed at enhancing the living, social and recreational skills of people with a disability. Community inclusion, independence, decision making, and personal choice should be promoted through these supports, with a focus on individual needs.

**ABOUT YOU**
- You are a highly motivated and compassionate individual who will be working with our participants who identify as LGBTIQ+ and assist with day to day activities.
- You are reliable and are able to work on a rotating roster including weekends and public holidays and inactive sleepover shifts
- Experience with mental health support work is highly regarded
- LGBTIQ+ Friendly

**ABOUT US**

Momentum Community Connections exists to encourage participation, promote inclusiveness, and create community connections by providing a wide range of support services and programs to Victorians aged between the ages of 17 and 65 living with disability to lead an independent and happy life.

**KEY RESPONSIBILITES**
- Provide assistance with households as outlined in the participants plan and encourage them to participate in those tasks actively
- Provide support that will enable participants to actively engage in the community or in any daily personal activities of their own choice and interest
- Maintain an environment which promotes participants decision making skills and contribute to their personal growth
- Show empathy and respect for the dignity, rights, and care of all the participants
- Participate in weekly team meetings and other administrative duties as requiired
- Community Access & Transport
- Maintain & Monitor Person Centred Plans
- Documenting & Progress Note keeping

**MANDATORY REQUIREMENTS**
- NDIS Screening Check (willing to obtain one)
- NDIS Orientation Modules and others
- WWCC- Employee
- Drivers Licence and car insurance
- Ability to commit to a rotating roster including in-active sleepover shifts
- Smart Phone
- Intermediate computer skills
- Excellent oral and written communication skills

**Job Type**: Casual

**Salary**: From $30.95 per hour
