**About the role**:
We are urgently seeking an **Experienced Social Worker **to join our fast paced and dynamic team. With a patient first focus, you will provide high quality care, that cares for the patient as a whole and involve them in their own care.

Location - All Sydney, NSW

**Job type - Casual (Part time). You will start on a part time basis initially and then gradually move into a full-time role.**

What you will be doing:

- Conduct assessments, develop care plans, and deliver therapeutic interventions.
- Collaborate with multidisciplinary teams to ensure holistic care for patients.
- Support families and caregivers, offering guidance and resources as needed.
- Advocate for patients' rights and ensure they receive appropriate community services post-discharge.
- Provide social worker support of all ages and cultural groups, including individuals, families and communities.

**What are we looking for**:

- Master’s degree in social work
- AASW Registration.
- 2+ years of work experience in clinical areas, including a demonstrated understanding of the health setting, complex health conditions and the impact on wellbeing.
- Demonstrated ability to initiate, lead, complete and share quality improvement activities and service evaluation processes with other staff and demonstrated capacity to provide clinical supervision.
- Demonstrated ability and experience in service planning and development and implementation of new models of care.
- Excellent communication, negotiation, liaison, advocacy and conflict resolution skills, and ability to work collaboratively in a multidisciplinary team.
- Demonstrated ability to work autonomously with mínimal direct supervision
- Demonstrated commitment to personal and professional development

**Mandatory Requirements**

You’ll have full Australian working rights.
- NDIS Worker Screening Check
- WWCC
- Valid National Police check
- First aid certificate (Asthma management and Anaphylaxis Management training approved)
- Driver’s licence and access to own vehicle would be advantageous
- Infection control training

Immediate start would highly be preferred.

**Competitive pay rates.**

**Job Types**: Part-time, Casual

Pay: $29.97 - $50.00 per hour

Expected hours: No less than 20 per week

Schedule:

- Afternoon shift
- Evening shift
- Morning shift
- Shift work
