**About the Role**:
As a Support Worker at Cessible Healthcare, your role is more than just providing care—it’s about making a real difference in the lives of participants. You will be assisting them in achieving their personal goals, enhancing their quality of life, and ensuring they remain in control of their care and decisions. You’ll foster meaningful relationships, actively listen to their needs, and deliver support that is flexible, respectful, and tailored to their evolving circumstances.
**Roles and Responsibilities**:
- Provide **individualised and flexible support** that adapts to each participant’s unique needs and aligns with their NDIS goals.
- **Uphold the principles of choice and control**, ensuring participants have an active voice in how their supports are delivered.
- Work collaboratively with participants and their support networks to develop and implement strategies that **promote independence and well-being**.
- Build and nurture **trusting relationships** with participants, advocating for their rights, dignity, and access to community services.
- **Encourage participation** in community activities, helping participants overcome obstacles and engage in meaningful, fulfilling experiences.
- Regularly **review and adjust care plans**, staying attuned to changes in participants' needs, preferences, and circumstances.
- Maintain clear and accurate documentation, reporting on progress towards NDIS goals and contributing to data collection for future care planning.
- **Collaborate with external agencies**, clinical services, and other providers to ensure seamless and coordinated care for participants.
- Engage in continuous professional development and **training to uphold the NDIS Code of Conduct** and ensure best practices in care delivery.
- Perform other reasonable duties, contributing to a **positive and supportive team culture**.
**Skills and Knowledge**:
- **Empathetic, patient, and supportive** nature with a genuine passion for helping participants achieve their goals.
- Excellent communication and interpersonal skills, with the ability to build rapport and trust with participants and their families.
- **Proactive and adaptable**, capable of adjusting supports to meet evolving participant needs.
- Strong **advocacy skills** to ensure participants’ rights and access to services are upheld.
- Ability to work effectively in a **team-oriented environment** while maintaining individual responsibility and initiative.
- Basic IT skills for data entry and reporting purposes.
- Physical capability to perform the role, including personal care and mobility assistance when required.
**Qualification and Requirements**:
- Worker’s Screening Check (NDIS Worker Check).
- National Police Check.
- Working With Children Check (if applicable).
- Certificate III in Individual Support (Disability, Aged Care) or a similar qualification is highly regarded.
- Previous experience in disability support or a related field (preferred but not essential).
- Current First Aid and CPR certification (required).
