The Organisation:
Our customer is a long standing member of the community in providing creative, planned and innovative youth focused services. This well-established organisation is a non-for-profit Brisbane Logan based service and provides a wide variety of youth based services.

The Role:
Our client is looking for a Youth Outreach Case Manager to work in our youth participation program, Youthlink. The Case Manager provides intensive case management support to at-risk young people aged 10 to 21 years linking them with support services tailored to their individual needs.

Our client is a leader in Youth Services in the Logan City Council, Jimboomba and Redland City Council including Moreton Bay Island. Our integrated approach is designed to deliver better outcomes for our clients. In this role, you will collaborate with other teams across our range of services including, alcohol and other drugs, transition from care, youth justice, housing and legal services. This also means there are great opportunities to develop your career.

**The role is permanent, full time, located in Slacks Creek.**

**Key Responsibilities**:

- Deliver effective case-managed services to at risk young people.
- Support young people to experience positive relationships with their family and community.
- Link young people into education, training and/or employment.
- Assist clients to access safe and stable places to live and lead healthy and violence free lives.
- Provide mental health interventions and assist clients to link with additional support services tailored to their needs.
- Work with parents and families, significant support people and stakeholders to build participation and independence.

**To Be Successful**:

- Minimum Diploma level qualifications relating to the community service sector, or higher.
- Proven experience in person-centred, case-management service delivery.
- Demonstrated knowledge and understanding of the factors contributing to young people disconnecting from family and community or disengaging from education, training and/or employment.
- Well-developed interpersonal communication skills to engage with a range of people (e.g. young people and families, education providers, community practitioners and specialist clinicians) including people from diverse cultural and socio-economic background.
- Effective written communication skills to write correspondence and prepare short reports
- Team work skills with the ability to contribute to a productive and harmonious team environment.
- Demonstrated organizational skills to plan and prioritize work efficiently and effectively, and to manage competing demands and priorities.
- Work out of hours when required to respond to client needs (e.g. to assist with before and after school routine and engagement with weekend events).

**Additional Requirements**:

- Current QLD Drivers License
- Current First Aid (or ability to obtain)
- Current Blue Card
- Current National Police Check
- Fully Vaccinated against COVID-19

**Salary and Benefits**

Salary is paid between **$77,832 to $81,915 **a year based on experience, with salary sacrificing available and a monthly accrued day off. Vehicles are available for work purposes.
