Reporting directly to the General Manager, the Team Leader - Home Care Package (HCP) and Private Home Care is to support the continued growth of Reliant Healthcare.
- Work with Clinical Service Manager for overall coordination of the Home Care Services division ensuring a high standard of service delivery appropriate to Reliant’s brand
- Manage Home Care Packages clients’ services
- Provide the Clinical Coordinator team with support, guidance, and direction, and help to develop good staff morale, staff skills and foster work productivity
- Participate in the development and implementation of operational plans for in conjunction with the General Manager
- Maintain and develop sustainable partnerships with both stakeholders
- Ensure all client information obtained is accurately entered into Reliant Healthcare data bases and client records in a timely manner.
- Ensure contracts are developed and regularly reviewed for each client as per the guidelines.
- Provide information to management related to new clients, level of packages signed up.
- Conducting home care assessments and formulating care plans, managing individual budgets, and monitoring and maintaining care in accordance with Home Care Standards
- Undertaking clinical assessments
- Assist in the development of policies, create reports, and provide information to the General Manager on the performance of the various programs
**To be suitable for this position you must have**:
- Qualifications in Home Care Package Case Management or Community Care Experience in clinical and case management in home care package setting
- Have a strong understanding and knowledge about the Australian Aged Care Standards, and Home Care Packages
- Demonstrated experience in case assessment
- Person-centre focused with exceptional time management skills
- High level relationship management skills
- High attention to detail and data accuracy
- Excellent customer service skills
- Great at solving problems
- Excellent computer literacy skills including proficiency with MS Office
- Demonstrated experience in supervising and leading a team
- Qualifications in Allied Health, or unrestricted registration with APHRA as an RN
