**The Organisation**
Sydney North Health Network (SNHN) is the Northern Sydney Primary Health Network (PHN), part of a national program of primary health networks designed to increase the efficiency and effectiveness of health care for patients, particularly those at risk of poor health outcomes.
Sydney North Health Network works with a broad range of stakeholders to transform the health system in the region by building synergies, streamlining approaches, and creating momentum for population health improvement.
Sydney North Health Network has adopted an innovative business model that is data-driven, using epidemiological information, evidence and local knowledge to set clear priorities for improving health outcomes, achieving better patient experiences and improving efficiencies.
**The Role**
Reporting to the Director of Commissioning and Planning, this role is key resource in ensuring that SNHN is a data driven organisation which uses the full range of evidence to set clear priorities and allocate resources to develop and commission primary health care services..
Specifically, you will:
- Lead a small team responsible for regional primary healthcare population health analysis, health intelligence, identification of priority needs and setting local performance indicators
- Identify, interpret, and analyse population-based data to inform regional population health needs
- Enhance the capacity of the PHN to use relevant data and evidence in health service planning and commissioning
- Leverage the entire body of available data to support service planning and commissioning, outcomes assessment, evidence-based care assessment, and evaluation
- Develop partnerships with Local Health Districts, universities, government agencies and other key stakeholders
**Skills Required**:
You will be a senior health professional with sophisticated program planning and relationship management expertise.
You will have:
- Previous experience in health-related data management applied to population needs assessment, strategic population health planning and/or program/service evaluation
- Experience in team leadership and supervision
- Demonstrated knowledge of the principles underpinning the population health approach.
- Superior experience working closely in collaboration with stakeholders
- The ability to present information persuasively to different audiences
- Tertiary qualifications in health services management, public/population health or associated field
