**Your Role & Responsibilities**:

- Provide **person-centered** and **active support** tailored to each participant’s goals.
- Assist with **daily living activities**, including personal care, mobility, meal preparation and household tasks.
- Support participants in **social and recreational activities** within the community.
- Implement **individualized care plans** and support strategies.
- Ensure a **safe, respectful and empowering environment** for participants.
- Maintain **accurate and timely documentation** of care and progress.

**What You Bring**:

- Evidence of working with **people with disabilities, their families and support networks**.
- A strong understanding of the **NDIS Quality and Safety framework** and **NDIS Code of Conduct**.
- Practical experience in **person-centered and active support models**.
- Ability to provide **support during overnight sleepover shifts**.
- Experience in **team collaboration, supervision and conflict resolution**.
- Strong **time management, administrative, and computer skills**.
- Excellent **communication and interpersonal skills** to build strong relationships with stakeholders.
- A **resilient and adaptable approach** to changing priorities and participant needs.

**Qualifications & Requirements**:
✔ **At least 5 plus years of disability support work experience.**
✔ **Certificate III in Individual Support (Disability)** or a willingness to obtain it.
✔ **NDIS Worker Screening Clearance** and **NDIS Orientation Module Completion**.
✔ **Current First Aid & CPR Certification**.
✔ **Working with Vulnerable People (WWVP) Check** with **NDIS endorsement**.
✔ **National Police Check (less than six months old)**.
✔ **A valid Australian driver’s license** (preferred).
✔ Availability for **flexible shifts, including overnight and weekends**.
