Homelessness Melbourne Metro is committed to providing person-centred care, trauma-informed practice and a strengths-based approach to improve the health, well-being and housing outcomes for people experiencing homelessness.
Service Profile:
The Homelessness Melbourne Metro region is a large Homelessness provider in the inner city of Melbourne and provides a comprehensive range of residential & community services across seven sites in St Kilda, Footscray, North Melbourne, West Melbourne, Kensington and Fitzroy.
Community programs include the Crisis Contact Centre, Community Outreach Services for single adults, support for older people in public housing, rough sleeper & street sleeping initiatives, community participation and AOD support.
Residential services operate from Foley House in Footscray, the Open Door in North Melbourne and Flagstaff in West Melbourne.
Homelessness Melbourne Metro is supported by co-located services including the TSA Chaplaincy program, Bolton Clarke primary health service and NDIS support coordination.

The position:
The Melbourne Street to Home (MS2H) service is led by Launch Housing and is delivered in partnership with The Salvation Army and Bolton Clarke to provide a multi-disciplinary team approach. MS2H provides housing and support to rough sleepers in the Melbourne CBD and surrounds. MS2H works with the highest acuity clients directly referred from the rough-sleeper teams, who identify rough-sleepers with a chronic history of homelessness and people with multiple and complex needs.
MS2H use an assertive outreach approach through intensive support to engage with people sleeping rough. MS2H provides coordinated housing, support services and health intervention. It is a Housing First program which aims to provide a rapid housing response and intensive support pre

- housing. Flexibility, persistence, creativity, and responsiveness are cornerstones of the MS2H model.
We are committed to working with the most disadvantaged, marginalised, and complex members of our community to support them in recovering from their experience of trauma and homelessness.
We are looking for an enthusiastic Case Manager who:
- Has experience working with rough sleepers or vulnerable clients who have multiple and complex needs
- Understands the range of needs and issues affecting people who are homeless or at risk of homelessness
- Is an effective communicator and natural relationship builder
- Works collaboratively with clients and other organisations to achieve common goals
- Takes accountability for your work outcomes and can form and maintain strong relationships
- Have outstanding time management skills with the ability to self-direct and work independently when needed
Essential Criteria:
- Relevant Bachelor or higher qualification in welfare or social work is required
- A valid and current Victorian driver's licence
