About the Role
The Aboriginal Health Care Coordinator is primarily responsible for providing direct clinical support to Identified First Nations and Non identified patients with health related issues through:
- Continuing to facilitate the successful integration of an Aboriginal health role within Health Independence Program (HIP) and Health care for the Homeless services at SVHM- Focusing on reducing the rate of hospital admission/readmission and presentation to the Emergency Department for First Nations clients with complex psychosocial and health issues, through the provision of care coordination including clinical services of engagement, assessment, care planning, support, referral and planning for discharge from the service- Improvement of linkages across the acute and primary health sector to improve access to services for First Nations clients with complex needs- Leading the development of cultural support, training and development of tools and resources in collaboration with Wilam Ngarrang to support the provision of culturally sensitive health care across the service.
The classifications for this position range from:
Registered Nurse Grade 4A YW17 ($104,816 per annum)
Allied health Grade 2 Social Work SC21 - SC24 ($83,361 - 95,825 per annum), or Occupational Therapy VF6 - VF9 ($83,361 - 95,825 per annum)
Your Contribution- A health professional with qualifications relevant to HIP (Social Work/Occupational Therapy/Nursing) minimum 4 years’ experience in relevant acute/community setting- Eligible for registration/membership with AHPRA or Discipline Specific Association (where applicable).- Of Aboriginal or Torres Strait Islander descent- Demonstrated knowledge of First Nations culture and values and social health issues- Experience in working constructively with First Nations communities and personnel- Demonstrated understanding of the Victorian Health System and Aboriginal Community Controlled Health Organisations- Current National criminal history check, or willing to obtain

ALERT Psychosocial Stream
This position will sit within the SVHM Healthcare for the Homeless program (HfH) multi-disciplinary ALERT Psychosocial stream.
ALERT aims to improve health outcomes for people who either frequently present, or who are at risk of presenting to hospital, because of chronic disease or complex medical and / or psychosocial issues.
The key target groups for ALERT include patients with complex needs who fall under one or more of the key risk factors; substance use, mental health issues, homelessness, disability and complex aged care issues.

This position integrates and works closely in planning, professional development, quality activities and research within the First Nations Services. First Nations Services drives self-determination, co-design and cultural safety for First Nations people across the organisation through policy reform, improvement projects, consultation and cultural support for staff and patients.
The Aboriginal Hospital Liaison Officer team within the First Nations Services works collaboratively with our medical teams to provide cultural social and emotional wellbeing support to our First Nations patients.
This position is responsible operationally to the Manager of ALERT: Healthcare for the Homeless and Cultural responsibilities with the Manager of the AHLO team.
