About the Role
- Support and liaise with hospital and community Chronic Disease services to coordinate health care service delivery and information for Aboriginal and Torres Strait Islander people and their families.
- Engage with the local Aboriginal and Torres Strait Islander community and relevant health and social services to identify priority actions to improve the health outcomes of Aboriginal and Torres Strait Islander people and their families.
- Deliver evidence based and culturally appropriate individual clinical services and group health promotion, education and Chronic Disease management activities to Aboriginal and Torres Strait Islander people and families.
- Promote the importance of physical activity, rest/sleep, being mentally healthy, good nutrition and reduction of preventable Chronic Disease risk factors (smoking, alcohol, obesity).
