Person Centred Support Worker

**About Me**
Hello I am a 62-year-old male with a psychosocial disability and acquired brain injury living in Glenside. Soon, I will be moving to Christie's Beach.

I am looking for self-reliant Support Workers to assist me in achieving my NDIS goals. I enjoy playing chess, listening to Jimi Hendrix, sports, cooking and having intellectual conversations.

**The Role**
An Opportunity is available to join my team of wonderful support workers who help to facilitate my daily life. My support workers assist me on a 2 to 1 service model throughout the day.

Due to some of the challenges I face, I have difficulties regulating my emotions and expressing myself. When I am feeling unsafe, I tend to react quickly. Please recognize my condition and understand my situation. In response, I would like you to be emphatic, provide space and show compassion.

I am seeking a Support Worker who is wanting to join a team environment, passionate about increasing my independence and advocating for my rights.

**Key responsibilities**:

- Assist me with ADL's
- Accessing the community
- In home activities
- Support me to achieve my goals to reach my potential in all facets of my life.
- Support my life skill development through proper implementation of established plans and programs to support the achievement of goal outcomes for me.
- Actively engage in person centred active support.
- Maintain accurate records.

**Shifts**
I am looking for someone to support me on a casual basis with the potential to join the team part time in the future.

My shifts are generally but not limited to; Monday, Wednesday, and Friday 1:30pm-3:30pm. However, over time, my hours of support will increase.
- NDIS Worker Screening or DHS Disability related check
- Current Driver's Licence
- Current First Aid Certificate
- Current CPR
- Safety Intervention training
- Certificate III in Individual Support (Disability/Aged Care) or other relevant qualification

**About you**:
I am looking for a respectful and positive person to join my team. A good match for me would be someone who has an efficient and confident approach in what they do and possesses a calm and patient nature. A good sense of humour is also important.

**Other requirements**:

- Experience in working with people who have experience with complex behaviours of concern's
- Ability to implement individualised developmental programs.
- Experience implementing positive behaviour support strategies.
- Flexible working hours
- Experience in various models of support preferred.
- Ability to adapt quickly to changes in service delivery.
