About the role
The Clinical Nurse Consultant (CNC) Cancer Care Coordinator (CCC) is an advanced practice registered nurse who provides support and nursing leadership within West Moreton HHS. In this role the Cancer Care Coordinator (CCC) will provide specialised Lung and Melanoma cancer information, education and support to patients with these cancers.
The CCC will be responsible and accountable for providing a high level of person-centred supportive care for patients with complex health needs and coordinate delivery of specialized support services across the care continuum to patients and their families. This encompasses patient assessment and coordination of care required, provision of education to patient, carer, and staff; consultation, provision of counselling to the patient and carer, and liaison and collaboration with multiple stakeholders to ensure the best patient outcomes.

About you
- Demonstrated excellence in communication and inter-personal skills across a variety of mediums and ability to build and maintain strong relationships, open communication, and a positive environment within a multidisciplinary team inclusive of external stakeholders.
- Proven ability to implement changes and develop direction in clinical practice by the utilisation of evidence based research and best practice principles
- Demonstrated skills in strategic and clinical service planning including an understanding of organisational behaviour, change management and cultural change processes.

**Mandatory Requirements**:

- Minimum five years full time equivalent post registration experience, with at least 3 years full time equivalent experience in Medical Oncology and extensive knowledge of chemotherapy, immunotherapy and targeted therapy including the processes involved in administration, side effects management and treatment pathways.
- Post Graduate Qualifications in Cancer Nursing or other qualification or experience in a similar field/s is essential.
- Vaccinated against, and remain vaccinated against, certain vaccine preventable diseases (VPD) as outlined in the Health Employment Directive NO. 01/16
