The Support Coordinator involves connecting participants with an NDIS plan to supports and services in the community. The main purpose is to support and assist within Australia’s NDIS participant’s abilities to connect to and coordinate informal, community and funded supports in the service delivery environment.

**Position Summary**
- Demonstrate high level interpersonal and communication skills
- Well-developed oral and written communication skills including problem solving and negotiation skills
- Commitment to the delivery of a quality participant service in balance with WHS principles, cultural diversity and equal employment opportunity
- Demonstrated understanding of the changing and diverse needs of home care participants
- Ability to work with a high level of independence and autonomy

**Desired Experiences**
- Tertiary qualifications in a relevant field, such as Social Work, Service Coordination, Case Management, Allied Health and/or Disability-specific Qualifications at associate diploma level
- Previous learning and development experience within Health, Disability or not for profit sector.

**Minimum Knowledge**

The Support Coordinator must understand the relevant legislative requirements, in particular
- NDIS Code of Conduct
- NDIS Legislation
- Understanding of NDIS
- Work Health and Safety Act 2012 (SA) (the WHS Act)
- Work Health and Safety Regulations 2012 (SA) (the WHS Regulations) and supported Codes of Practice
- Equal Opportunity Principals and Practices Practice
- Ability to record and maintain accurate non-judgemental information

**Personal abilities/Aptitudes/Skills**
- Demonstrated high level interpersonal/communication and analytical skills
- Commitment to the delivery of a quality participant service in balance with WHS principles
- Established organisational ability to manage busy day to day administrative activities
- Willingness to work in a busy and challenging environment and build rapport over the phone
- An understanding of cultural sensitivity
- Proven experience in the use of Microsoft Office including Outlook, Word and Excel
- Proven capability to allocate time effectively and efficiently
- Demonstrated understanding of the changing and diverse needs of home care participants
- Ability to process and prioritise a high volume of activities in a fast-paced environment
- Demonstrated ability to use a planned, structured approach to allocation of resources
- Ability to work with a high level of independence and autonomy

**Support Coordinator/Roles and Responsibilities**

The roles and responsibilities of a Support Coordinator may include, but are not limited to the following:

- Work with participants to find providers, services and support that work together to achieve their goals in line with their NDIS plan
- Resolve points of crisis and develop resilience in a participant’s network
- Involve participant and family in the implementation of individual packages where appropriate
- Identify and respond to the learning needs of individuals and the services
- Actively participate in Professional Development
- Ensure that information on relevant community resources and all service provision options are available to particip
