Your role will involve helping clients with daily living activities, fostering independence, and promoting their well-being through personalised care plans. *Key Responsibilities:*
- Provide one-on-one support to clients in their homes, community settings, or transitional housing facilities.
- Assist clients with personal care tasks such as bathing, dressing, grooming, and medication management.
- Support clients in household tasks including meal preparation, light housekeeping, and errands.
- Engage clients in meaningful activities and social interactions to enhance their quality of life.
- Monitor and document clients' progress, behaviors, and responses to interventions.
- Collaborate effectively with therapists, healthcare professionals, and family members to ensure coordinated care. *Qualifications:*
- Compassionate and empathetic approach to supporting individuals with diverse mental health and disability needs.
- Previous experience in caregiving, support work, or related fields is preferred.
- Ability to communicate effectively and build rapport with clients, families, and multidisciplinary teams.
- Reliable and flexible availability, including evenings, weekends, and holidays as needed.
