**Salary**: $86,165 - 108,187 per annum plus superannuation
**The opportunity**:
The Project Coordinator (Transition Compass Coach) will support a national clinical trial investigating Transition Compass—an innovative intervention designed to assist young people with chronic health conditions as they move from paediatric to adult healthcare.
In this pivotal role, you'll provide one-on-one support to trial participants and work closely with the Transition Compass research team to ensure the smooth delivery of the trial across multiple sites. You will be involved in recruitment, onboarding, digital engagement, and helping young people overcome barriers to accessing adult healthcare services.
**Your responsibilities include**:
Deliver individualised support to young people participating in the Transition Compass trial
Recruit and consent eligible participants in line with ethical and research protocols
Guide participants through the digital platform, track progress, and escalate support when needed
Collaborate with clinical and transition teams across paediatric and adult services
Maintain meticulous records and uphold research data integrity
Facilitate peer support sessions and stakeholder meetings
**What we need from you**:
A tertiary qualification in nursing, psychology, allied health, or a health science field
Experience in clinical care or research, ideally within adolescent or chronic health contexts
Excellent interpersonal, communication, and organisational skills
A strong understanding of the Australian healthcare system and transitions in care
Competency using IT systems including electronic medical records, research databases (e.g. REDCap), and mobile apps
