**Salem Home Care** is part of the Salem and Northridge Salem supported living communities, operated by Lutheran Services and has been serving the Toowoomba region for more than 80 years.

**About the role**:
The role of our Home Care Manager is to deliver the Home Care Packages program within the local community to ensure a positive outcome for Clients. This role will see you leading the coordination of business processes to ensure compliance with organisational standards and regulatory bodies including effective training and education around care planning, goal setting and budget management. You will monitor and review the delivery of services to meet our client’s individual need and undertake regular risk assessments with a continuous improvement approach.

You will seek regular feedback from Clients on Service provision and promote and represent the organisation to a range of external stakeholders to ensure that Lutheran Services is recognised as a leading provider of aged care services.

**About you**:
You will have exceptional experience in a similar role with a proven track record of meeting objectives and timelines. You will have a passion for promoting independence and self determination by older people. Committed to collaborative partnerships, you are skilled at building and drawing on mutually beneficial stakeholder relationships at all levels.
