**Key Responsibilities**:

- Conduct comprehensive evaluations of clients' physical, emotional, and social requirements.
- Collaborate with clients, their families, and healthcare professionals to gather pertinent information essential to the assessment process.
- Develop and execute personalised care plans based on assessment findings.
- Conduct regular reviews and updates of care plans to reflect changes in clients' needs and preferences accurately.
- Coordinate and arrange a diverse range of in-home support services, encompassing personal care, nursing, therapy, and allied health services.
- Establish effective communication with external service providers to guarantee the seamless delivery of care.
- Act as an advocate for clients by ensuring that their rights, preferences, and choices are respected and upheld.
- Regularly oversee clients' progress and adjust care plans as necessary.
- Perform routine assessments to gauge the efficacy of the care provided and offer suggestions for enhancements.
- Maintain precise and current records of client assessments, care plans, and ongoing communications. Additionally, produce reports as necessary for both internal and external stakeholders.
- Prepare reports as needed for both internal and external stakeholders.

**Qualifications and Experience**:
Preferably, a bachelor’s degree in nursing with a current APHRA registration or a related field is preferred.

Prior experience in case management within the aged care sector is required.

A comprehensive understanding of the Australian aged care system and portals is essential.

Exceptional communication, interpersonal, and problem-solving skills.

The role also demands empathy, patience, and a genuine dedication to enhancing the lives of aged care clients.

**Job Types**: Full-time, Part-time, Permanent

Pay: Up to $108,680.00 per year
