We provide a range of services through the South Burnett area with teams operating from Kingaroy, Wondai, Nanango and Murgon.

**Program Summary**

South Burnett CTC Inc (CTC) is a registered NDIS provider dedicated to empowering people with disabilities in the South Burnett region. CTC Disability Services provides services participants across the South Burnett including Support Coordination, Plan Management and Frontline Services. Our commitment to quality care, as overseen by the NDIS Quality and Safeguards Commission, ensures that participants receive personalised support plans tailored to their needs. Through services like personal care and assistive technology, participants exercise choice and control, fostering independence and community engagement. Upholding the principles of inclusion and respect, we strive to create an environment where individuals thrive with access to necessary services and opportunities.

**Position Summary**

As an integral part of our team, you will play a vital role delivering exception support and care to our participants. Working collaboratively, you will ensure that all goals outlined in individual participant’s NDIS Plan are actively worked towards. Your dedication to providing quality support within the framework of the NDIS Code of Practice is essential in empowering individuals with disabilities to thrive and achieve their aspirations.

**Attributes Requires to Meet the Responsibilities of the Position**
- Qualifications/Industry Experience/Licences _
- Certificate III in Individual Support (Disability) or other relevant qualification desirable
- Experience in the delivery of personal care services and/or challenging behaviours
- Commitment to empowering individuals with disabilities
- Ability to adhere to standards set by the NDIS Quality and Safeguards Commission
- Flexibility and adaptability to the needs of participants
- Ability to collaborate with colleagues, participants, and support networks to support participants to meet their goals
- Commitment to promote respect and inclusion for persons living with a disability
- Effective communication skills with stakeholders
- Demonstrated compassion and empathy towards participants
- Willingness to engage in continuous learning and professional development
- Queensland Open Driver’s Licence
- First Aid and CPR Certificates or ability to acquire
- A reliable, registered private vehicle, covered by current comprehensive insurance, suitable for transporting participants and/or participant visits
- NDIS Worker Orientation Module “Quality, Safety and You” Screening
- Working with Children Blue Card
- NDIS Worker Screening Check
- Ability to meet and maintain any other current or future legislative benchmark requirements and government or organisation mandates Cultural Awareness
- Ability to respond in a culturally sensitive way to all stakeholders of the service
- Ability to uphold cultural protocols and maintain community support Time Management and Accountability
- Time management and IT competency to ensure case notes and other required documents are captured on a database system Availability and Flexibility
- Flexibility to adjust hours when required to meet the needs of participants

**Responsibilities of the Position**:

- Client Services: _
- Facilitate participant’s access to community activities and outings, ensuring engagement and participation in line with their NDIS Plan and Support Plan
- Provide direct care assistance to each participant in alignment with their Support Plan
- Adhere to medication administration policy, safety procedures, and participant rights policies
- Monitor the safety and well-being of participants and promptly reporting concerns to the Team Leader
- Ensure compliance with necessary documentation, recording support information accurately
- Deliver high-quality care and support to all participants
- Provide personal care with empathy and respect
- Ensure compliance with organisational policies and procedures
- Collaborate with participants and professionals to support the achievement of client goals
- Operational:_
- Accurately clock-in and clock-out of every shift
- Review participant general shift notes prior to every shift
- Complete a general shift note for every participant, for every shift, reflecting on the participant’s goals and objectives within 24 hours of completion of the shift
- Complete incident reports as necessary, as soon as possible (within 24 hours) of the incident, ensuring details are accurately recorded
- Actively participate in staff meetings
- Understand and adhere to the NDIS Quality Safeguarding Framework
