**Position **Overview**
The Disability Support Worker is responsible for providing high-quality, responsive and person-centred support as part of a team to assist residents to achieve their goals, enhance their quality of life and physical and mental health in a Supported Independent Living environment.
**Key Responsibilities**
Provide services to meet the individual needs of people with disability, which may include:
- Transferring and hoisting (within given safety parameters)
- Personal care (grooming, dressing, toileting and showering)
- Domestic duties (meal preparation, shopping, housekeeping and cleaning)
- Medication management and administration
- Behaviour support for psychosocial clients
- Skills development training and social support
- Keep up to date with participants needs and care plans, including changes to participants care needs.
- Facilitate community inclusion and participation, dependent on individual needs, abilities and preferences, and support participants to establish and maintain relationships and interests through access to community activities and services.
- Provide regular feedback to the Practice Lead/Operations Manager/On Call Manager of issues that require follow up relating to the residents, their families and external supports, other UFC employees and community access activities.
- Ensure all participant documentation is current and up to date including the completion of end of shift case notes and shift handover processes.
- Work as part of a team following direction from the Operations Manager/ Practice Lead to achieve the best outcomes for the residents and reputation for UFC.
- Attend and participate in team meetings and staff development days.
- Complete mandatory online training in a timely manner.
- Maintain knowledge and work within UFC’s Code of Conduct, policies and procedures in the Quality Management System.
- Be aware of safe working conditions and implement safe working practices in all work areas and report potential hazards, incidents and concerns in a professional and timely manner.
**Position Description**
**Key Performance Indicators**
- **High quality participant support** - Provision of high quality participant support - participant’s needs are met as detailed in their support plans.
- **Documentation** - Complete all forms, data collection and shift notes per shift as specified for each participants individualised needs including incident reporting in alignment with UFC and NDIS requirements.
- **Contribution to team** - Participate in team based processes, including effective shift handover, de-briefing, team meetings and training sessions working collaboratively and respectfully with colleagues.
- **Self-Development** Undertake mandatory training and actively participate in self development activities.
- **Confidentiality** - The dignity of clients and their confidential information is maintained.
- **Compliance** - Adherence to UFC’s Code of Conduct, policies and procedures.
**WHS** - Demonstrate commitment and active contribution to the care, safety and wellbeing of self, participants and others to maintain safe working environment.
**Qualifications, Experience and Skills**
- Qualification in disability, mental health, or related field (desirable)
- Preferable 2 years experience supporting people in a related sector including with a disability, mental illness, community services, aged care, residential or rehabilitation environment.
- Current First Aid Certificate with CPR.
- Experience supporting participants with high needs
- Values consistent with person-centred ways of working that supports the person’s choice and control.
- A clear understanding of professional boundaries and restrictive practices
- Demonstrated ability to set priorities, meet deadlines, work with mínimal supervision and under pressure.
- Effective written and verbal communication skills and a demonstrated ability to write clear case notes.
- Strong interpersonal skills to collaborate as part of a diverse team and work autonomously
**Other Requirements**
- Flexibility and availability to work a 24/7 roster including morning, evening, overnight, sleepover shifts.
- National Police Check (obtained within previous 6 months)
- NDIS Worker Screening Clearance
- Unrestricted drivers licence and own vehicle with comprehensive car insurance
