**THE ROLE**

As a Youth Outreach Case Manager, you will be responsible for delivering outreach and holistic case management support for at-risk and vulnerable young people who are homeless or at risk of homelessness across the Frankston, Mornington Peninsula, Dandenong and Casey areas.

You will contribute towards enhanced outcomes for young people within their own communities, focusing on access and navigation to services and other support initiatives, early intervention and prevention strategies and links to broader health, welfare, education, employment and alcohol and other drugs (A&OD) service systems.

**ABOUT YOU**

You are passionate about working with and supporting young people, you see the unique potential and perspective they bring, and you deeply understand the relationship between education, community and family in bringing out their full potential. As a Youth Outreach Case Manager, you will:

- Provide high quality youth support services for a caseload of young people including intake and assessment, advocacy, case management, outreach and supported referrals.
- Work with young people to develop individual case plans within a case management framework and utilizing a strengths-based approach.
- Establish and maintain strong engagement with young people throughout the case management cycle.
- Support young people with budgeting needs and life skills as well as empowering them to identify education and employment pathways with the objective of moving towards a healthier, safer and positive lifestyle.
- Work in partnership with key external stakeholders, service providers and local community groups to identify service gaps and develop appropriate responses appropriate to the region
- Ensure best practice case notes and documentation is delivered including monthly program reporting and evaluation processes.
- Contribute to ongoing program and individual practice improvements through attendance at internal Communities of Practice and reflective practice opportunities.

**Qualifications**
- Relevant tertiary qualifications or have demonstrated experience in education, youth work, social work, social sciences (or similar).
- Training in relevant child protection legislation.
- A current working with children check, and driver's license (and willingness to undergo a Police Check process)
