
Discharge Facilitator

About the role
The purpose of this role is to work collaboratively and consultatively across multidisciplinary mental health services to actively support and promote discharge planning processes on the ward. This comprises facilitation of key discharge planning and intervention programs.
About you
- Demonstrated knowledge and skills in contemporary clinical practice within the area of mental health with a particular focus on navigating inter-agency systems.
- Demonstrated understanding of the principles, processes and environments that support recovery and an ability to work within a Recovery Framework.
- Demonstrated communication and team participation skills including the ability to build, maintain and effectively manage professional relationships and influence a range of stakeholders to promote positive consumer outcomes.
- Demonstrated experience in the identification, development and implementation of quality and service improvement and research activities to improve consumer care, commensurate with level of experience and under the clinical practice and / or professional supervision of a more experienced practitioner.
- Demonstrated ability to provide supervision, support, guidance and education to less experienced practitioners, students and members of the multidisciplinary team commensurate with level of experience.
**Mandatory Requirements**:
- Vaccinated against, and remain vaccinated against, certain vaccine preventable diseases (VPD) as outlined in the Health Employment Directive NO. 01/16
