**About the Role**
As a Case Worker your role is to provide direct support to individuals who are participating in the Community Living Support (CLS) program. You will ensure that the values of Open Minds and change principles from Motivational Interviewing and Acceptance and Commitment models are used as the framework for service provision. The role requires active engagement with colleagues, team leaders and other providers of services to ensure effective service delivery, and the reporting of service outcomes.

Creating and develop relationships with clients, their family, your colleagues, and teams from other agencies
- By developing a shared understanding about your clients’ values and helping them take actions that are consistent with these
- Working with the people to promote self-resilience and independence
- Assisting people to make connection with community and local resources
- Timely and accurate communication to Team Leaders, colleagues, and partner agencies particularly relating to blocks to progress, adverse events, and any changes to risk.
- Active participation in monthly one to one, peer reflection and all reporting requirements
**Who you are**:
- Education and qualifications._
- Relevant Degree or Diploma (Mental Health, Community Sector, Case
Management, Counselling or related)
- Essential skills, experience, attitude_
- Curious, open, and self-reflective person
- Enthusiastic about learning new approaches to mental health work and willing to actively engage in continuous improvement opportunities
- Significant experience working in team base models within the health, mental health, community, or social service sector
- Strong verbal and written communication, negotiation & conflict resolution
skills and the ability to engage with people with mental health difficulties, their
families and other service providers and partner agencies.
- Capacity to work independently and collaboratively being accountable for the effective delivery of services.
- Strong understand about job and professional boundaries
- Licences/Checks_- Current Driving Licence
- Current Working with Children Check
- Current Criminal Record Check.
- Up to date COVID vaccinations
