**The role**
As an ECH Care Coordinator you will work closely with our Home Care Package Clients, their families, carer(s), and advocates. You will ensure all clients have a Care Plan that reflects their individual care and services needs to enable them to have the best life possible as they age in their own home.
ECH Care Coordinators are responsive to their clients ever changing needs. You will spend most of your time visiting, consulting with, and coordinating services for clients, inclusive of client budgeting, within your case portfolio. You will be provided with full training and support including all equipment that is required to succeed in this role. **You will also have the option for a subsidised fully maintained vehicle.**

**What you’ll be doing**
Under the guidance of our operational and clinical service leadership team, our Care Coordinators:
- Provide advice and guidance to enable informed decision making and self-determination
- Undertake comprehensive assessments of client needs to maximise their HCP budget and to ensure individualised goals are developed into a Care Plan that enable them to continue to live independently within the community.
- Identify when a client’s needs have changed to ensure they continue to receive appropriate care and advocate to support their increasing needs.
- Develop effective networks to support ECH business growth and targets whilst promoting ECH services and products that enable clients to achieve their goals.

**You will need**
- COVID-19 vaccination (including booster if eligible)
- Drivers licence
- Relevant tertiary qualifications in health, social or behavioural sciences, and /or extensive care coordination experience in the health & community sector.
**It’d also be helpful if you had**:
- A current Police Check
- Experience and/or knowledge of Home Care Packages
**The salary band for this position is **$73,412 to $78,891 dependent on qualification and experience.**

ECH is a not-for-profit organisation in the Health Care Sector. This means that you can salary package up to $18,549 per annum to reduce your tax. We also offer a range of perks through our Employee Benefits Program (eg. discounted gym memberships).
You will be provided with full training and support including all equipment that is required to succeed in this role, with the option for a subsidised fully maintained vehicle.
