**Looking for**:
We are seeking a passionate and dedicated Community Worker to join our team. The Community Worker will play a crucial role in ensuring the effective and efficient delivery of our services to individuals with disabilities. This role involves working closely with clients, families, and support teams to coordinate and implement personalized support plans.

**Roles & Responsibilities**:

- Work closely with clients to understand their unique needs, preferences, and goals. Develop and implement personalized support plans that focus on enhancing independence, well-being, and social inclusion.
- Support clients with daily living activities, including personal care, meal preparation, and household tasks, ensuring a respectful and dignified approach to care.
- Encourage and assist clients in participating in community activities, social events, and recreational programs. Help them build connections, access local resources, and develop their social skills and confidence.
- Conduct regular assessments and interviews to identify client challenges and track their progress toward achieving their goals.
- Assess community needs and resources to design, develop, and implement programs that address local health, housing, education, welfare, and employment issues.
- Offer education and care for children and individuals with disabilities in adult service units, group homes, and government institutions, while collaborating with families to promote a nurturing environment.
- Regularly assess clients' progress towards their goals, identify potential challenges, and adjust support strategies as needed. Provide feedback to both clients and the management team to ensure continuous improvement in service delivery.
- Support young individuals in navigating social, emotional, and financial issues, helping them find effective solutions and build resilience.
- Maintain detailed and accurate records of client interactions, progress notes, and any changes in care plans. Ensure all documentation complies with company policies and legal requirements.
- Act as a liaison between clients and other service providers, community organizations, and government agencies.
- Prepare detailed submissions, reports, and funding proposals for government bodies and agencies. Ensure that programs and services have the necessary resources to meet community needs.
- Ensure the health, safety, and well-being of clients by following appropriate protocols and responding to any emergencies or incidents in a calm and professional manner.

**Skills and Attributes: -**
- Diploma, Advance diploma or Bachelor's degree in a related field.
- Minimum 2 years’ experience working in community services or a similar field.
- Knowledge of relevant legislation and policies related to community services.
- Strong communication and interpersonal skills.
- Ability to work collaboratively with clients, families, and a multidisciplinary team.
- Empathy, patience, and a genuine commitment to promoting the rights and well-being of individuals with disabilities.

**Job Type & Salary: -**
- This is a full-time permanent job.
- The base salary will be AUD 75,000- AUD 80,000 plus superannuation.

**Working Hours: -**
- Availability to work during evenings, weekends, and holidays as required.
