Comfort Home Care has a vacancy for an experienced and reliable Home Care Manager to assist our participants in their homes.

Comfort Home Care provide in home care to our clients 7 days a week.

The Care Manager role is responsible for ensuring quality care and support to enable more older Australians to remain living at home safely. You will be responsible for the planning and delivery of timely care and support that meets participants goals and delivers exceptional services.

You will work with participants and their families to develop customer care plans while providing professional, high-quality support in a safe manner.

You will have an excellent focus on compliance and documentation, understanding the regulatory responsibilities.

You will work as part of a dynamic, passionate and fun team in a fast-growing business.

**Care Manager Key Responsibilities**
- Interact with healthcare providers to facilitate participants’ treatments
- Ensure participants’ treatment requirements are met
- Suggest alternative treatment plans when participants’ services requests do not meet medical necessity criteria
- Instruct and educate customers on procedures, healthcare provider instructions, and referrals
- Link participants to social services programs and entitlements such as transportation assistance and translation services
- Conduct regular follow-ups with participants to evaluate progress, promote continuity of care, and ensure improved health outcomes
- Maintain records of case management activities

**REQUIREMENTS AND SKILLS**
- Proven work experience as a Care Manager or similar role
- Proficient with Healthcare Management Systems and Microsoft Office Word and Excel and E Tolls
- Outstanding communication skills, both written and verbal
- Excellent organisational skills
- Ability to travel locally
- Relevant training and/or certifications as a Care Manage_r_

**Essential**
- Own vehicle and Current Driver’s License (C class).
- Police checks, employment history and background checks.
- COVID-19 & Flu vaccinations.

**Job Types**: Full-time, Casual

**Salary**: $20.33 - $37.74 per hour
