**About the role**:
At BaptistCare HopeStreet, we seek to transform the lives of people in our communities who are living with disadvantage or experiencing great personal trials.

The Pathways Leader will:

- Coordinate and support the team to provide advice, referral pathways and case management support to clients.
- Coordinate and support the team to provide an integrated casework and case management service to clients who are vulnerable and require assistance and specialist services, escalating where required.
- Degree or Diploma in Community Services, Counselling, Social Work or related field
- Minimum of three (3) years working in a Case Worker role
- Current NSW Drivers Licence
- Hold a current Working with Children Check
- Hold a current First Aid Certificate or be willing to obtain a first aid certificate
- Experience working with and understanding the issues impacting on Aboriginal and Torres Strait Islander peoples

Desirable qualifications / experiences:

- Mental Health First Aid accreditation, Trauma Informed Care training, Child Protection Training.
- Financial management and understanding budget Understanding of Collective Impact
