**About the department**:

- The Social Work department is committed to promoting the highest level of care for our patients. In realising this goal, the department leads and excels in person-centred care, evidence-based practice, innovation and quality improvement. The department provides Social Work services on all three campuses of Austin Health, from the Emergency Department through to bed-based services and ongoing ambulatory care.

**About the role**:

- As a Grade 1 Social Worker you will be responsible for providing clinical social work services to inpatients across one or more of our Austin Health sites. A regular day will see you work clinically with patients, attend family meetings and non-clinical meetings, and undertake other duties that support the Social Work department and its operations (as appropriate for Grade 1).

**Key responsibilities will see you**:

- Undertake comprehensive psychosocial assessments, and develop and implement appropriate intervention plans, including care planning, counselling, and discharge planning for patients and their families.
- Participate in quality improvement, service improvement and/or research

**About you**:

- You will be someone who has excellent communication skills and a strong commitment to excellence in patient care. You’ll bring energy and drive to you work, and have a keen interest in developing your skillset.
- You’ll further have:

- A degree in Social Work
- The ability to work and contribute to team culture and work within a multidisciplinary environment
