Northern Adelaide Local Health Network (NALHN) is eager to recruit to a newly created Social Worker role within the dynamic, expanding and supportive Geriatrics in the Home team.

**About the Role**:
An exciting opportunity is presented to work within the Northern Adelaide Local Health Network as an AHP2 Social Worker in the Northern Adelaide Geriatric Service.

This new position has been created to provide targeted social work assessment and intervention within the expanding Geriatrics in the Home (GITH) service. Geriatrics in the Home (GITH) is a hospital substitution service, which offers an alternative approach to delivering in-patient care and treatment to Older People within their own home environment. The service offers specialist assessment and therapy from Geriatricians, Nurses, and Allied Health in a goal orientated, time limited program.
