An established not-for-profit provider of Aged Care in the NSW South Coast, this organisation have been providing residential, retirement and community aged care services to the community for more than 50 years. With an expanding Home Services portfolio, they are seeking a Clinical Manager to lead the Clinical aspect of this service line.

**About the Role**

Reporting to the General Manager (Villages & Home Services), the Clinical Manager (Home Services) will lead a team of Registered and Enrolled Nurses to ensure the effective delivery of clinical and lifestyle support to residents in their homes, whilst upholding the organisation’s model of care.

The Clinical Manager, Home Services, will be responsible for (but not limited to) the following:

- Demonstrating knowledge and understanding of customers’ needs/wants in consulting with relevant stakeholders, facilitating customer choices and making decision within their area of expertise where required.
- Overseeing the development of comprehensive care and support plans for customers based on assessment outcomes, individual needs and preferences and medical and allied health advice and directives.
- Consulting with Registered Nurses and other members of the team, including GPs / NPs, specialist services and allied health providers to facilitate quality clinical care to customers.
- Coordinating monthly relevant clinical audits and completion of evaluations reporting results to the Leadership meeting monthly.
- Lead by example to develop, support and guide the Clinical Care (Home Services) Team.
- Ensure compliance with all relevant WHS standards

**About You**
- Bachelor of Nursing or equivalent with current registration within the Australian Health Practitioner Regulation Agency
- Proven track record in delivering quality clinical care
- Excellent clinical assessment skills
- Proven success in case management
- Experience in effectively leading a team
- Knowledge of Home Care Package Guidelines and Aged Care Quality Standards
- Understanding and commitment to workplace equity issues.
