**About the role**:
As a Case Manager, you will play a vital role within our welcoming and supportive Aged Care Team. You'll oversee the care of our Spectrum Home Care Package clients. Your dedication, compassion and connection to your clients will make a profound difference in their lives. Your thoughtful assessment and sound judgement will ensure that your clients feel safe and supported in their homes, and enjoy living their lives to the fullest.

**To do this role, you will need**:

- Tertiary qualification in relevant fields such as Social Work, Nursing or Allied Health.
- 2+ years’ experience in case management, including working with complex cases.
- Knowledge and experience in the Aged Care sector.
