9D Care is a NDIS registered service provider of in home and community support across Australia. 9D Care offers a range of supports and assistance to a variety of residents, including help at home, social inclusion and community connectedness. We are looking for people who can provide practical and emotional help to our participants by consulting with them and determining their needs, allocating suitable support workers and coordinating participant care services for the Northern & Western suburbs of the Greater Melbourne region. A Care Coordinator will also be working with the care team to evaluate interventions and improvise our current processes.

Successful applicants will display a compassionate nature, be knowledgeable about health care practices, and provide exceptional customer service.
- **Responsibilities**:_
- Onboarding new participants and identifying their needs
- Communicating between staff, participants, and family
- Building and maintaining relationships with key stakeholders
- Recruiting and training staff in line with the company policies and procedures
- Workforce Management and Rostering
- Developing participant goals, monitoring progress, and taking regular feedback
- Maintaining participant confidentiality and quality care
- **Requirements**:_
- Degree/Diploma/Certificate in Social Work, Mental Health, Community Services, Individual Support, Nursing or a related field
- First Aid & CPR Certification
- Excellent verbal and written communication
- Ability to manage multiple tasks concurrently & meet deadlines
- Knowledge of working on a CRM/Rostering system will be an added advantage
- Computer skills and proficiency in Microsoft Office
- Ability to work in a fast-paced environment
- Ability to travel (car preferred)

**Job Types**: Permanent, Full-time

**Salary**: $60,000.00 - $65,000.00 per year
