**About the role**

The primary objective of the role is to plan, co-ordinate and provide day to day administration of Home Care Packages; ensure the activities of the Home Care Packages operate effectively and efficiently to meet the identified needs of frail elderly people & their carers.
- **Key Responsibilities**_

Your main responsibilities will include but not be limited to:

- Initial and ongoing assessment of service users’ needs
- Develop, implement, monitor and review service user care plans in consultation with clients
- Provide case management support to service users that have a person-centred focus.
- Managing financial aspects of service users’ budgets including monthly statements
- To coordinate services and activities of the Home Care Packages and provide direct care when necessary.
- Responsible for the supervision of care service delivered by Home Care Employees, and complying with documentation requirements
- Provide information, advice and referrals to all people seeking information about Home Care Services and other aged care support services
- Undertake After Hours On Call duties

**About the Successful Applicant**
- Essential:_
- Minimum 3 years of working experience in aged or community-based services for older people
- Tertiary qualifications in aged, disability or related areas e.g. Nursing, Community, Health, Welfare or attained Certificate IV in Care Support Services / Aged Care, case management
- Demonstrated ability to work independently and work well with others
- Excellent interpersonal and time management skills
- **Speak fluent Cantonese/Mandarin and good command of English and good report-writing skills**:

- First Aid Certificate or willingness to obtain one
- Current full driver's license
- National Police Clearance
- Australian Citizenship / Permanent Residency
- Desirable:_
- Experience in Home Care Services /dementia care/respite care
- Experience in case management
- Experience in leading a team
- Experience in working in a community-based setting
- Experience in working with Culturally and Linguistically Diverse populations
