**About the role**

We are seeking a dedicated and compassionate **Social Worker** to join our dynamic **Community Mental Health Team**. In this vital role, you will provide high-quality mental health support and interventions as part of a multidisciplinary team. You will work closely with individuals facing complex challenges, including mental illness, substance abuse, and homelessness, offering tailored support to help them achieve their recovery goals.

**Your key responsibilities will include**:
Supporting individuals with multiple complex needs, including mental health, substance use, and housing instability.Working as a case manager as part of the community mental health team.Working within a recovery framework to empower individuals, their families, carers, and community networks to make positive changes.Collaborating with healthcare professionals, service providers, and community organizations to create a holistic care plan that meets each person’s unique needs.Providing a compassionate, person-cantered approach that focuses on the dignity, autonomy, and well-being of each individual.

**About you**Current Registration with the Australian Health Professionals Regulation Agency (AHPRA) or Australian Association of Social Workers.Extensive experience working within a Community Mental Health role as part of a multidisciplinary health care teamCurrent unrestricted NSW Drivers License to facilitate community-based support.Proven ability to provide high-quality consumer care, including support for families and carers of individuals with mental illness.Commitment to recovery-focused care, with the ability to deliver holistic mental health interventions that support individual and community well-being.
