CARE MODE PTY LTD is a NDIS disability services provider located in Footscray, Melbourne. We support people with disabilities to achieve their goals in their homes, and within the community. Our approach is person-centred, and we are a "lifestyle-focused" care organization. We service all metropolitan Melbourne and have support workers located across all suburbs who comes from a variety of different qualifications, backgrounds and cover around 23 languages. We are seeking for a full-time, **Disabilities Services Officer **to join our team in Melbourne. **Your responsibilities will include**: 1. Meeting with clients to assess their circumstances and needs and develop support programs to ensure client needs and expectations are met. 2. Design programs to help clients with their financial, emotional, and societal factors. 3. Monitor and review client progress and coordinate with them to ensure progress as well as maintaining client support plans. 4. Supporting clients by providing appropriate education and care including making monthly home visits and/or as per client request. 5. Liaising with internal team members to resolve client issues. 6. Directing clients towards organizations that can provide them with additional assistance they need and liaising with appropriate agencies. 7. Create and maintain efficient systems and workflow and maintain client records. 8. Ensure adherence to pertinent policies and procedures. - Bachelor level qualification - At least 2 years of relevant work experience in the field - NDIS awareness, particularly in relation to client services management and the importance of choice and control for participants - Demonstrated ability to develop positive relationships with a range of participants, their families, and their service providers. - Well-developed communication (both oral and written) skills with the ability to prepare reports and correspondence in clear and concise language. - Strong interpersonal skills with an understanding of diversity when working with participants and colleagues. **Salary**: $35.00 - $40.00 per hour **Benefits**: - Visa sponsorship Schedule: - 8 hour shift Ability to commute/relocate: - Melbourne, VIC: Reliably commute or planning to relocate before starting work (required) Licence/Certification: - First Aid Certification (preferred) - Working with Children Check (preferred) - National Police Certificate (preferred) - Driver Licence (preferred) Work Authorisation: - Australia (preferred) Work Location: In person
