**The Opportunity**:
Yerin Eleanor Duncan is looking for two suitably qualified fulltime Aboriginal Transition of Care Workers to join our Yadhaba Team. The Aboriginal Transition of Care Worker role aims to support and empower Aboriginal individuals and communities throughout their mental health and AOD healthcare journey, ensuring culturally responsive and appropriate care during transitions between different healthcare settings. This position plays a vital role in bridging the gaps between mental health and AOD healthcare services and Aboriginal patients, enhancing their overall healthcare experience, and improving health outcomes.

**Essential Criteria**:

- Relevant qualifications to this role
- Experience within the mental health or AOD sector
