**NDIS Care Coordinator**

**Location**: Western Melbourne (office + community visits)
**Salary**: $60,000 - $70,000 + super | Full-time

**What You’ll Do**
- Welcome new participants through **intake and onboarding**, ensuring NDIS compliance.
- Develop, review, and update **care plans** in collaboration with families, support coordinators, and providers.
- Act as the **key contact** for participants, addressing concerns and ensuring high-quality service delivery.
- Conduct **regular client visits** to build relationships and monitor progress.
- Maintain **accurate records**and reporting for compliance and audits.
- Collaborate with **Finance, Admin, and Rostering Service Teams** to deliver seamless participant care.

**What We’re Looking For**
- Experience in **NDIS services, case management, or care coordination** (preferred).
- Strong communication and problem-solving skills.
- Ability to balance admin work with **face-to-face participant visits**.
- Knowledge of NDIS guidelines (or willingness to learn with training).
- Located in or willing to travel to Western Melbourne.

**The Details**
- **Full-time role** (38 hrs per week)
- **Pay**: $60,000 - $70,000 per year + super

Pay: $60,000.00 - $70,000.00 per year
