**The role**

In these Local Area Coordinator roles, working in the Northam location, you will be responsible for assisting people with disability, their families, and their carers to build and pursue their goal to live their best life, exercise choice and control, and engage with the National Disability Insurance Scheme - Partners in Community.
- You will be responsible for:_- Collaborating with people with a disability to assess their eligibility for supports through an NDIS plan.
- Connecting people with disabilities to supports within the local community
- Improving community knowledge of the NDIS and creating new opportunities to connect participants to services and support
- Meeting identified contractual KPIs

You are an approachable, positive, passionate, and respectful person who can build and nurture relationships easily and confidently. Strong personal values based on empathy and compassion, you understand the diverse needs of individuals within your local community and can coach, counsel, and connect people to identify and achieve the positive outcomes they desire.
- You will have:_- Demonstrated relevant experience and/or lived experience
- Experience in supporting people to achieve collaborative outcomes
- Knowledge of the local community and the available options for social & economic participation for people with disability
- Demonstrated computer skills including capability with Microsoft Office 365
- Demonstrated ability to work within a performance based culture
- Currently hold or able to obtain a current Working with Children Card and / or vulnerable people check; PEP and NDIS Worker Screening Clearance; and successful completion of NDIA entry level checks.
