About the opportunity

The objectives of the PAST Clinician is to:

- Achieve safe, coordinated and streamlined transition for clients from an acute mental health inpatient setting to the community.
- Provide clinical input to maximize the likelihood of developing strong connections with relevant supports within the community and minimizing an early readmission (within 28 days).
- Build strong partnerships with other clinicians within and outside of Mercy Health who are working to achieve the same goal.

Qualifications:

- An approved degree in Psychology and registration with AHPRA as a Psychologist with endorsement as a Clinical Psychologist or
- An approved degree in Occupational Therapy and registration with AHPRA as an Occupational Therapist or
- An approved degree in Social Work and eligibility for membership of the Australian Association of Social Workers
- A current Victorian Driver’s License and ability to drive a work vehicle

You will also be required to provide evidence of, or in the process of obtaining:
- A current Police Record Check and Working with Children Check.- Current COVID-19 certificate, proof of current influenza vaccination and other vaccination status consistent with public health orders.
