**Are you a motivated Case Manager looking to be part of our dedicated Home Care team?**

This is an exciting opportunity for a Case Manager to join the Respect Home Care Team. The role is based at our office in Hobart, Tasmania. This is a fast paced and varied role, working closely with our community service workers to ensure exceptional delivery of our in-home care services. You will be responsible for the provision of quality Home Care services to clients through assessment, case-management and coordination.

**This will be on a three-month contract, with the possibility of being extended, on a full-time basis (Monday to Friday).**

**Requirements**:
**What you can bring to the role**:

- Desirable Certificate IV in Individual Support
- **Relevant experience in Community Care within the Aged Care/Disability sectors**:

- **Experience in case management, leading and supporting services**:

- Excellent knowledge of Consumer Directed Care principles and aged care legislation
- Be self-directed to manage time frames and expectations
- Have highly developed interpersonal, communication and conflict resolution skills along with the ability to manage a diverse range of staff, contractors, clients and stakeholders
- Be creative in problem-solving skills, for developing solutions and for making sound recommendations based on commercial and risk based decision making
