Care Coordinators work closely with our Care Managers to help deliver quality care services that meet clients goals. Care Coordination is a great role to explore if Care Management is a career goal.

Care Coordinators and Care Managers go together like copy + paste. Each Care Coordinator directly supports several Care Managers by providing essential administrative support allowing the Care Managers to focus on care planning, management and review. As a Care Coordinator you must develop positive, trusting relationships with clients and liaise on their behalf with third party stakeholders.

A typical day could involve:

- Purchasing client equipment and supplies
- Entering Care Plans and maintaining customer records in our databases
- Diary management and meeting preparations on behalf of Care Managers
- Preparing client communications, contracts, budgets and journals
- Supporting clients allied health and medical needs by making referrals, booking appointments, requesting feedback and managing our Approved Service Providers compliance paperwork
- Auditing electronic files and managing compliance requirements

The role is fast-paced and requires you to be detail orientated, accurate and customer centric. You must drive a positive Client Experience, bringing your personality and passion to the role and build great relationships with your Care Managers.

**Skills & Experience**

To be considered for this role you must have/be:

- Must be an Australian Citizen or Permanent Resident.
- Excellent communication skills and in particular a first class telephone manner.
- Strong organisational and computer skills, high attention to detail and demonstrated initiative.
- Reliable, punctual, professional and a solid work ethic.
- A demonstrated empathy to the needs of older people.
- An outgoing personality; we're a friendly team that like to enjoy their time at work.
- A quick learner and a hard worker.
- Able to prioritize effectively and have good time management skills.
- Customer service centric and focused on the Client Experience.
- A clear Police Check.
- Experience with MS Office.

**Salary**: $45,000.00 - $50,000.00 per year
