**About the role**
The Care Coordinator is responsible for providing advanced clinical support, leadership and coordination of clinical care for residents with complex needs. You will provide mentorship and guidance for a care team of PCW's and Registered Nurses within our facility.
In this role you will:
- Oversee and co-ordinate the completion of focus assessments of individual resident needs and record findings within a Positive Wellbeing Model;
- Ensure that care plans are developed and reviewed based on the assessment findings of individual resident needs;
- Co-ordinate the collection of accurate clinical/care data for timely submission of Aged Care Funding Instrument;
- Monitor that resident documentation is accurately reflective of care provisions and is in accordance with organisational requirements;
- Co-ordinate the care provision of external service providers;
- Assist in the review and maintenance of clinical programs and initiatives;
- Co-ordinate the collection of clinical indicator data in accordance with the organisation's reporting timelines.
**_ In this position you will be working on the floor Monday - Friday, PM shifts._**

As the Care Coordinator, ideally your experience and qualifications include:
- Current AHPRA registration as a Registered Nurse, with min 2 years of aged care experience;
- Experience in managing and leading teams in the aged care sector;
- Strong knowledge of the aged care accreditation system and compliance;
- Demonstrated knowledge of AN-ACC;
- Experience in educating, mentoring and motivating staff;
- Superior communication skills both written and verbal;
- Be empathic, compassionate and committed to Resident Centric Model of Care.
