**FlexiHome Care** provides a wide range of in-home and community care support services to disabled people of all ages. We provide flexible disability support services that are centred on the client's needs and want. We provide our participant services beyond what is expected from us as we want to give them the best life they can achieve within our capability.

**The role**:
The role of the Support Coordinator is to support National Disability Insurance Scheme (NDIS) Participants to achieve their NDIS plan goals, by coordinating informal supports, mainstream supports and funded supports.

**Your key responsibilities will include**:

- Develop and maintain effective, collaborative relationships with key stakeholders that result in effective interactions, mínimal service delivery issues and appropriate referrals. Key stakeholders include, but are not limited to: Clients and their families, carers and/or guardians, NDIS service providers, NDIS
- Assist NDIS Participants to identify and plan strategies to achieve their personal goals within a person-centred framework
- Support clients to be implement their NDIS plan to maximise the value for money they receive from their supports
- Exercise participant choice, control assist and coach participants on how to utilise the NDIS participant portal to make the service bookings and changes
- Support Participants in facilitating their independence by enabling empowerment and choice, and the promotion of personal resilience and social inclusion. Community engagement and education
- Conduct community presentations to raise awareness of the NDIS, including people with a disability in mainstream supports
- Build and maintain relationships with internal and external stakeholders
- Actively participate and engage in team meetings and initiatives
- Assist with problem solving and address any emergent issues
- Identify any communication or workflow issues and makes suggestions for improvement Legislative and Policy Compliance
- Operates within relevant legislation, and NDIA policies and guidelines
- Prepare plan review reports and other requested reports

Support Coordination Administration
- Maintain client data base which may include but not limited to, soft and hard copy client files, progress notes and activities, NDIS billing costs etc.
- Complete a range of internal and external reports relating to clients including case management statistics and feedback
- Complete administrative tasks including required reports and referral letters within agreed timeframes
- Other duties within the scope of the role and skills of the incumbent as directed by the Team Leader

Ensure all workflow documents are followed and provide feedback to the Team Leader to contribute any updates to reflect the correct procedures and processes

**Ideal Applicants will have**:

- Experience as a Support Coordinator
- Knowledge and experience of NDIS funded plans
- Experience in Community Services/Mental Health/Disability or other Allied Health disciplines
- Evidence of COVID-19 Vaccination Status
- A strong commitment to a person-centred approach
- A commitment to maintaining confidentiality and ability to understand and maintain professional boundaries
- Understanding of the concepts of duty of care and dignity of risk
- Strong sense of teamwork and ability to work collaboratively
- A Drivers License

**Qualifications**
- Academic qualifications in health care, disability, nursing or any other relevant field.

**Experience**
- Experience as a Support Coordinator and managing NDIS funds
- Experience in disability, aged care or nursing sector**.**
