**About the Role**
We are seeking experienced & passionate Team Leader to provide care and support to achieve NDIS goals in line with Client's NDIS Support Plan in the community, SIL
This role independently supervises house staff to provide support to people with disabilities, across their life cycle and in all domains of life, to facilitate a high-quality life with dignity and respect in the least restrictive environment.
The successful applicant must demonstrates strong leadership qualities and be experienced in working with clients in Respite/ Group home setting. Working with a variety of clients some with complex behaviours, and mental health support needs, you will also have strong knowledge of relevant legislation and NDIS support knowledge as well as excellent skills in communicating effectively with a diverse group of key stakeholders. Team leader will also be scheduling shifts for staff and assist staff with incident reports and reports so a high level of leadership is required.
The Team Leader will assist in ensuring:
- The business is properly resourced for proper service provision
- That participants are supported to access services provided by Apex Community Carers.
- That participant right's policies including privacy and confidentiality are in place and adhered to by staff under their control.
- That the safety and well-being of the participants and the support workers is at the forefront of staff work.
- The completion of appropriate documentation. Relevant information on support provided is recorded in participant notes as required.
- They participate in Apex Community Carers’s planning process for individual participants and monitor outcomes.
- They work with participants, family/ carers, relevant case managers, support workers and other professionals to support the participant's individual goals.
- That all service provision undertaken is appropriately maintained, documented, and reviewed.
- They liaise extensively with other staff to ensure the overall smooth and consistent running of the business.
- They report regularly to the Coordinator.
- They actively contribute to a team environment through open communication, participation in regular staff meetings, planning processes and policy review.
- They participate in Performance Development meetings to identify ongoing training and support opportunities.
- They contribute to Case reviews with other staff, facilitators, Coordinators, and other interested parties.
- They assist with the management of the allocation of support workers to individual participants as needed.
**Key Performance Indicators**
- Shortages allowed, the business is well resourced, and staff can provide scheduled services
- Help to ensure:
- All participants receive a high level of care and support as per their plan.
- Personal care is provided to participants in a caring and supportive way.
- Participant privacy is always protected.
- Documented evidence demonstrates that service procedures are followed.
- All policies and procedures are followed.
- Records demonstrate that participants have received correct medical and dietary intakes as prescribed by medical practitioners and as per service policy and procedures.
- Participant files are updated and reviewed, and health care and behaviour support plans are in place as required.
- Daily documentation completed (i.e. diaries and daily statistics) as per procedures.
- A high level of communication with the Coordinator is maintained including advising of any potential risks to participants or the organisation.
- Induction, WHS, Manual Handling, Medication and Infection Control training are undertaken on commencement.
- Ensuring all rostered shifts are covered.
- Employee code of conduct is adhered to.
**Qualifications and Personal Qualities**
Hold a recognised Office Management Course certificate or documented previous experience. Diploma Qualification in community services or similar relevant field

- Commitment to social inclusion and valuing difference and diversity.
- Working with Kids check
- Current CPR Certificate (or willing to obtain)Current First Aid Certificate (or willing to obtain)
- Unencumbered Driver's license
- Current NDIS Worker Screen Check
- Police check
- Completion of the NDIS Worker Module
- Fully vaccinated against Covid-19
**Job Types**: Full-time, Part-time
Part-time hours: 38 per week
**Salary**: $60,000.00 - $65,000.00 per year
