**About the role**
- Competitive hourly rate, ranging from $38.88 - $39.71 depending on experience +11.5% SUPER
- Providing person-centred support for participants with high levels of complexity
- Review the care plan with relevant parties at least twice a year or at a time agreed upon by the client representative(s).
- Foster and sustain professional relationships with the client's representative(s) and family as a whole.
- Weekly 1:1 with participants to maintain the channels of communication
- Prepare support worker rosters in advance, ensure they are displayed at each site, and upload them into the Carelink system
- Undertake coaching, mentoring, and supervisions / performance management for staff as required
- Direct morning support including, personal care, medication administration, meal assistance, bowel care, and manual handling.

**About you**
- Experience providing diverse care to vulnerable individuals within the Disability Sector
- Hold a Certificate IV in Aged Care/Disability or Community Services and updated level 2 First Aid
- Experience in leading and supervising a team of support workers in SIL homes.
- Experience working with individuals with Cerebral palsy, Frontal lobe and Spina bifida highly regarded
- Excellent written, verbal and interpersonal communication skills
- Knowledge of funding NDIS
- Experience in developing and implementing individualized Care/ Support Plans
- Holds a valid NDIS screening check and Working with Children Check (WWCC)
