**Position Summary**

Peninsula Regional Assessment Service (RAS) is seeking to employ an enthusiastic and motivated Home Support Assessor to complement our existing multi-disciplinary team at Mt. Eliza Aged Care Assessment Service (MEACAS).
Practicing within the framework of My Aged Care and within Peninsula Health organisational requirements, the Home Support Assessor will provide a thorough and independent entry level assessment of the aged client and with the client develop care plans that promote the restoration of function, Re-ablement and/or maximise independence. The Home Support Assessor will formulate and review support plans and facilitate referrals for Commonwealth Home Support (CHSP) services to enable older people to engage the range of community resources in order to meet their specific needs.

**Key Responsibilities**

The position is available on a Fixed Term Part Time basis; previous community health, assessment or aged care experience are therefore highly desirable.

**Essential Criteria**
- Valid Victorian Driver’s License
- Computer and typing literacy
- Excellent verbal and written communication skills
- Commitment to delivering a first class service to the aged client group
- Develop and maintain essential working relationships with community providers and associated stakeholders
- Registration with AHPRA as required
- Previous RAS and community aged care assessment experience preferred
