Austin Health is the major provider of tertiary health services, health professional education and research across 3 campuses in the northeast of Melbourne and state-wide. The Austin Drug & Alcohol Service (ADAS) is funded by the Department of Health to provide Specialist Pharmacotherapy services. ADAS facilitates holistic, continuous clinical care (via specialist pharmacotherapy services and other clinically relevant interventions) for the treatment and management for clients with substance abuse issues. It supports the client as they move through the AOD system and also supports them to access other AOD services as required.

Objectives of the case manager role include to utilise therapeutic interventions to provide harm minimisation education, facilitate continuity of care and improve equity of access to treatment by prioritising vulnerable and at-risk clients. The case manager will conduct assessments, treatment planning and goal setting, and support referral into appropriate services. The case manager is expected to provide individualised and flexible support that supplements other AOD treatment services over a longer period (as per the model of care). The duration and intensity of the service should vary depending on a person’s need.

We are currently seeking** **a highly experienced and enthusiastic **Case Manager (SW2/OT2/RN3/P2**)** **with relevant experience in the Drug and Alcohol sector** **to take on a permanent part-time position (64 hours per fortnight) working a set day roster.

Applicants will be required to demonstrate a good understanding of recovery oriented care, the Victorian Drug and Alcohol sector (including the Victorian Pharmacotherapy system and relevant legislations), and will have a demonstrated ability to work within a multidisciplinary team and provide clinical guidance and support to their colleagues. The successful clinician will provide case primary case management to a group of identified complex consumers in establishing recovery goals and building on their personal strengths through skills training, education, and other evidence based therapeutic approaches. They will also provide support to consumers of other case managers as required.
