**About The Mental Health Division**
- The Mental Health Division (MHD) provides mental health assessment and interventions for people experiencing mental illness and their families and carers. Working from a recovery-oriented model, the Mental Health Division provides an array of hospital-based, community and specialist services for infants, children, young people, adults and older adults.

**About the role**
- CCS is a multidisciplinary team of highly passionate, experienced, supportive, collaborative and enthusiastic mental health professionals. Our values underpin everything we do. You will form solid working-relationships within the pre-existing community teams and foster strong therapeutic relationships with your clients & their families/carers. You’ll join a friendly and supportive team who genuinely enjoy their work and the contribution they make.
- A regular day will see you:

- Assess and treat our clients, liaising as appropriate with family members and caregivers to promote excellent clinical care
- Collaborate with multiple teams across the division and wider community
- Support the care and recovery of peoples living with complex mental health diagnoses and presentations
- Advocate for your clients right to access safe and timely care, at the right place at the right time

- As a Case Manager, You will be someone who has excellent communication skills, with a strong commitment to excellence in patient care. You’ll bring energy and drive to your work, and have a keen interest in developing your skillset.
- To be successful in this role, you'll bring with you:

- A commitment to working in a strong team & a desire to support & grow with your colleagues
- Demonstrated knowledge of recovery and collaborative clinical practice.
- A working knowledge of the Mental Health & Wellbeing Act (2022), the Carer Recognition Act (2012), and Chief Psychiatrists Guideline
