As a registered NDIS provider, New Horizons specialise in supporting people with disability, mental health concerns, those who are aged, people at risk of homelessness, humanitarian entrants, youth, and Indigenous Australians.

You will be the first point of contact for NDIS participants, families and Support Coordinators who are seeking information about New Horizons or who have chosen us to provide services for them under an NDIS plan.

Based in **Gosford**, the role is **Permanent part-time Support Worker position** with rotating shifts providing the highest quality in high needs / complex care disability services to the community.

**The hours of work vary from weekday shifts, and weekend shift.**

These positions support the delivery of Community based support that is funded by the National Disability Insurance Scheme.

**Key responsibilities will include**
- Supporting NDIS participants to access group activities and events organised through New Horizons including participation and facilitation of groups (where required)
- Providing a person-centred, recovery-oriented approach to participants
- Contributing information for the development of customer support plans
- Providing support to NDIS participants in terms of developing independent living skills, transport, social activities, prompting with medication and other support as required
- Facilitation of psychoeducation and skill-building
- Keeping participant records up to date on a daily basis using online systems
- Developing and maintain professional relationships and networks with customers, families, caregivers and key stakeholders

- Competent in working with people with a disability

**What you need to have**
- Appropriate certificate level qualification in Disability or Community Services, OR equivalent relevant experience in the human services industry
- Valid NSW Drivers C class licence, with ability to drive vehicles in licence class, including large cars and vans.
- Must be fit to undertake the physical requirements of customer support, including the capacity to lift 10 kilograms unassisted
- Intermediate skills in Microsoft Office, other common systems
- Passion, Respect & Integrity
