Our multi-disciplinary team includes Child and Family Health Nurses, Social Workers, a Paediatrician, and a Speech Pathologist, working together to provide holistic, compassionate, and person-centred care.
**About the Role**:
In collaboration with social workers, you will offer case management support, including conducting home visits, monitoring child development, providing guidance, and referring families to services such as speech therapy or occupational therapy when necessary.

We are seeking a compassionate and dedicated professional with post-graduate qualifications in Child and Family Health Nursing and extensive experience in this field. You will bring excellent educational and support skills, along with the ability to empower families to set and achieve their goals.
